A-Delta Fibers, C-Fibers and Chronic Pain: How Scrambler Therapy Helps Change the Message

First, a Quick Clarification About “S-Fibers”
People sometimes use the term “S-fibers” informally when talking about sensory nerves. However, S-fibers are not a standard scientific category of pain fibers.
The primary nerve fibers involved in pain signaling are A-delta fibers and C-fibers. Another group, called A-beta fibers, normally carries non-painful information such as touch, pressure and vibration.

A-Delta Fibers: The Fast Warning System
A-delta fibers are thinly covered with myelin, an insulating layer that helps electrical signals travel quickly. They are responsible for the immediate, sharp and more localized pain you may feel when you step on something sharp or touch a hot surface.

“Something just happened, pay attention now.”
C-Fibers: The Slower, Lingering Pain Pathway
C-fibers do not have a myelin coating, so they conduct signals more slowly. They are commonly associated with pain that feels:
- Burning
- Aching
- Throbbing
- Diffuse or difficult to pinpoint
- Persistent after the initial injury
C-fibers can respond to mechanical, thermal and chemical changes. They are also closely connected to inflammatory and autonomic responses.
This is one reason chronic neuropathic conditions may involve much more than pain alone. A person may also experience temperature changes, swelling, skin-color changes, sweating changes or extreme sensitivity.
What Changes When Pain Becomes Chronic?
Acute pain serves an important purpose: it warns us of actual or potential tissue damage.
Neuropathic pain, however, comes from a lesion or disease affecting the somatosensory nervous system. When pain persists, peripheral nerves, the spinal cord and the brain may become increasingly responsive to incoming signals, a process often described as sensitization.
The nervous system can begin to interpret input as dangerous even when no new injury is occurring. Normal touch may be experienced as pain, known as allodynia, and a painful stimulus may feel far more intense than expected, known as hyperalgesia.
This does not mean the pain is imaginary.
The pain is real. It means the nervous system’s processing of sensory information has changed.
How Scrambler Therapy Works

The device generates changing electrical waveforms intended to mimic non-pain information. Instead of simply blocking a pain signal or creating a distracting sensation, the proposed goal is to send the nervous system a different message:
“This area is not in danger.”
During a successful session, the painful sensation may be replaced by a comfortable buzzing, tapping or vibration-like feeling.
Accurate electrode placement, appropriate intensity and continuous patient feedback are essential because Scrambler Therapy is highly technique-dependent.
Researchers propose that these synthetic non-pain signals travel through the same broad sensory communication system involved in the painful area, including C-fiber pathways. With repeated exposure, the central nervous system may learn to interpret the input differenty, reducing pain and, in spaome, allodynia.
The exact biological mechanism is still being studied. Scrambler Therapy should therefore be described as neuromodulation, changing how the nervous system processes information, not as permanently repairing every damaged nerve or guaranteeing a cure.
Why Sessions Are Performed on Consecutive Days
Chronic pain patterns usually developed over time and changing them may require repeated, consistent input. Scrambler Therapy is commonly delivered as a series of sessions on consecutive or near-consecutive days rather than as an occasional treatment.
The goal is not simply to create temporary comfort during a session. It is to repeatedly reinforce a non-pain message so that relief may last progressively longer between sessions.
Responses vary. Some people notice a meaningful change quickly, others need more sessions, and some do not respond.
Where Red and Near-Infrared Light Therapy May Fit
Red and near-infrared light therapy, also called photobiomodulation, works differently from Scrambler Therapy. It does not program a synthetic sensory message into the nerves. Instead, light absorbed by cells may influence mitochondrial activity, circulation and inflammatory signaling.
Research suggests photobiomodulation may help reduce pain in some conditions and studies of peripheral neuropathy have reported promising findings involving pain, sensation or nerve function. However, protocols vary widely by wavelength, dose, treatment area and diagnosis, and the evidence is not equally strong for every type of neuropathic pain. It is important to work with a Certified Light Therapist when incorporating light therapy into the protocol.
When used alongside Scrambler Therapy, red and near-infrared light should be viewed as a supportive modality. Scrambler Therapy targets pain-information processing, while photobiomodulation may support the local tissue and cellular environment. They are different tools that may complement one another, not interchangeable treatments.
Why Nervous System Regulation Matters
Persistent pain and a constantly activated stress response can reinforce one another. Pain can increase fear, muscle guarding, poor sleep and sympathetic “fight-or-flight” activity. Those changes may then increase sensitivity and make pain harder to settle.
Nervous system regulation does not mean telling someone to relax or suggesting that pain is caused by stress. It means creating conditions that help the brain and body feel safer while treatment is occurring.
Slow breathing, comfortable positioning, hydration, sleep support, gentle movement when tolerated and noninvasive vagus nerve stimulation may help reduce overall arousal and improve a person’s ability to receive sensory input without feeling threatened.
For conditions with autonomic features, such as CRPS, this whole-system perspective can be especially important.
Scrambler Therapy addresses the pain message, while supportive regulation strategies may help calm the environment in which that message is being processed.
A Combined Approach to a Complicated Problem
Chronic neuropathic pain rarely involves only one structure or process. It can include abnormal signaling in peripheral nerves, sensitization within the spinal cord and brain, inflammatory activity, disrupted sleep, reduced movement and an overprotective stress response.
That is why a thoughtful plan may combine:

The aim is not to dismiss or cover up pain. It is to give the nervous system clearer, safer input and a better opportunity to change an established pain pattern.
